New patient forms

Thank you for setting up an appointment to visit with us about your healthcare needs. Below you will find the four forms we need filled out before we can see you. If you have access to a printer, print out these four forms and fill them out before you come for your first visit. Please arrive 15 minutes ahead of your scheduled appointment time to allow time for paperwork to be entered into our software. Also, please take some time to look through the chiropractic side of our website, especially the YouTube video Dr Amy did explaining Atlas Orthogonal Chiropractic. Directions to our clinic in Ankeny, Iowa are also on the website. Your first visit will be an hour long. Drive safe and we look forward to meeting you!
